Patient Care Specialist for Older Adults and Stroke Unit

We are seeking a compassionate and skilled Patient Care Specialist to join our team in the Older Adults and Stroke Unit at Trinity Health. As a Patient Care Specialist, you will play a vital role in providing high-quality patient care, ensuring the comfort, safety, and well-being of our patients.
Job Summary:The Patient Care Specialist will work under the direction of a registered nurse to provide direct patient care, considering the age-specific, developmental, cultural, and spiritual needs of our patients. You will assist in establishing and maintaining a patient care environment that is clean, safe, and conducive to patient/family well-being.
Responsibilities:- Assist patients with completing ADLs, including bathing, showering, toileting, positioning, turning, transferring, ambulation using assistive devices, and feeding and menu selection.
- Collect and send specimens, including blood and blood cultures, and insert and remove peripheral IV catheters.
- Record patient information and required data in appropriate areas to meet documentation requirements.
- Work in a medical progressive care unit specializing in the care of diagnoses, including renal, stroke, seizure, and COVID-19 patients.
- High school diploma or GED.
- Minimum of one year of previous direct care experience (acute or long-term) or successful completion of CENA, PCT, MA, or EMT course.
- Student nurses must have completed their nursing fundamentals class of a BSN program.
