Job Summary

We are seeking a Registered Nurse to join our team at Shared Services Home Health Care and Shared Care Services. As a Registered Nurse, you will provide high-quality care to our clients in the comfort of their own homes. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each client.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Conduct thorough assessments of clients' physical and emotional needs, and develop and implement individualized care plans.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Monitor clients' progress, and make adjustments to care plans as needed.
  • Document all interactions with clients and their families, and maintain accurate and up-to-date records.
Requirements
  • Current RN licensure in the state of Michigan.
  • At least 1 year of experience in home health care or a related field.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift 25 pounds and stand for long periods.
