Position Summary

The Non-Acute Registered Nurse serves as a vital caregiver for a designated patient demographic, focusing on achieving optimal health outcomes. This role emphasizes the enhancement of clinical knowledge and skills, delivering effective direct care as part of a multidisciplinary team to manage a diverse range of complex patient needs. The nurse will supervise and guide the care provided by other team members through delegation, validation, evaluation, and coordination of services.

Key Responsibilities

A typical day for a Non-Acute Registered Nurse at Hackensack Meridian Health includes:

Assessment:

  • Gathering comprehensive data relevant to the patient's health status.
  • Conducting age and condition-specific assessments independently and systematically, focusing on physiological, psychological, and cognitive aspects.
  • Identifying changes in clinical and diagnostic status.
  • Recognizing educational needs of patients and families based on age, condition, and cultural learning differences.

Planning:

  • Creating a care plan that outlines strategies and interventions to achieve expected health outcomes.
  • Formulating patient and family-centered goals based on assessment findings.
  • Coordinating a care plan that is focused on the patient and family, prioritized, and encompasses the entire continuum of care.

Implementation:

  • Communicating, coordinating, and executing the care plan effectively.
  • Maintaining ongoing communication with patients, families, nursing colleagues, and other healthcare team members regarding patient status and care plans.
  • Delivering age and condition-specific care in an organized and timely manner.
  • Providing or coordinating patient and family education as necessary to ensure understanding.
  • Adhering to established protocols in clinical situations, including effective communication and documentation.
  • Administering nursing care and treatments that reflect patient needs, advocating appropriately, and demonstrating evidence-based practice.
  • Conducting continuous electronic documentation of each patient's assessments, care plans, interventions, and evaluations.
  • Utilizing advanced medication administration technologies to ensure safe delivery of medications.
  • Facilitating multidisciplinary discharges to ensure safe transitions and continuity of care.

Evaluation:

  • Assessing progress towards achieving health outcomes.
  • Applying critical thinking skills based on nursing knowledge to meet desired patient outcomes.
  • Initiating interventions in response to changes in patient condition and unexpected reactions to care.

Additional Responsibilities:

  • Identifying ethical issues in practice and addressing them with the team.
  • Recognizing opportunities to enhance patient satisfaction.
  • Maintaining competencies, certifications, and continuing education to meet accreditation and licensing requirements.
  • Adhering to the American Nurses Association Code for Nurses.
  • Meeting physical demands of the role, including lifting and standing for extended periods.
  • Complying with organizational competencies and standards of behavior.
  • Performing other duties and projects as assigned.

Qualifications

Required Education, Knowledge, Skills, and Abilities:

  • Progress towards a BSN or willingness to obtain within three years of hire.
  • A quick learner with the ability to adapt to changing patient needs, a strong sense of accountability, and a commitment to delivering high-quality patient care.

Preferred Education, Knowledge, Skills, and Abilities:

  • National Certification in specialty area preferred when eligible.

Licenses and Certifications Required:

  • New Jersey State Professional Registered Nurse License.
  • BLS/CPR certification by the American Heart Association required within 60 days of hire.
