Job Title:
LPN Coordinator

Job Summary:
This position is responsible for managing patient care, promoting effective education, and ensuring timely healthcare delivery. The LPN Coordinator will serve as a liaison between residents, families, physicians, and other healthcare providers. The ideal candidate will have a strong understanding of OSHA and state regulations, as well as excellent communication and interpersonal skills.

Key Responsibilities:
• Serve as a liaison between residents, families, physicians, and other healthcare providers
• Maintain a current knowledge of OSHA and state regulations
• Provide follow-up contact with patients to ensure compliance with recommendations
• Educate and involve patients to improve their health-patient engagement
• Assist patients to set goals, progress, and identify barriers when patients are not meeting treatment goals
• Manage aspects of patient care, including referrals, hospitalizations, ER visits, ancillary testing, and other as directed to achieve optimum outcomes in a safe and cost-effective manner
• Develop a list of medical supply and community resources available to patients
• Operate department(s) within established budgetary guidelines
• Coordinate continuity of care with the patient and/or family to ensure compliance
• Provide clinical follow-up with patients, giving information and guidance to enhance patient-care team communication
• Maintain accurate and timely documentation

Education and/or Experience:
Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
