We are seeking a skilled Clinical Liaison to serve as an advocate for patients during the pre-admission process. As a key member of our team, you will work closely with clinical teams and healthcare professionals to evaluate patients' medical needs and develop results-producing relationships throughout the medical facilities.
Key Responsibilities:
- Perform clinical assessments of referred patients and counsel with patient's families to assist them in making the best care decisions for their loved one.
- Partner with hospital case managers to create smooth patient transfer processes and help remove payer-related and operational barriers to getting appropriate patients into this highly effective level of care.
- Develop and maintain relationships with healthcare professionals and patients to ensure seamless transitions of care.
Requirements:
- Registered Nurse (RN) or Respiratory Therapist (RT) License.
- Prior sales or business development experience is preferred.
- Strong critical thinking skills and problem-solving abilities.
