Job Summary:

The Patient Care Coordinator will work collaboratively with physicians, nurses, and other clinical staff to ensure seamless patient care. This includes reviewing charts, flagging pending health maintenance and treatment issues, and following up on missing referral documentation and laboratory work.

Responsibilities:
  • Coordinate clinic sessions and support managing the clinic flow
  • Assist providers during procedures, obtain patient informed consent, and provide before and/or aftercare instructions
  • Administer vaccines, non-intravenous injections, and perform venipunctures as directed by a licensed provider
  • Collect blood samples from patients using age-appropriate venipuncture or capillary puncture and following LCDP policies
  • Perform on-site laboratory services and patient procedures, including simple dressing changes, urine dipsticks, finger/heel sticks, A1C, EKG's, adult/pediatric immunizations, injections, and venipuncture
  • Document all follow-up activities and interactions in the patient's medical record
  • Participate in regular meetings with the supervisor, team huddles, triads, case review meetings, and staff meetings
Requirements:
  • High School Diploma or General Education Development (GED)
  • Bilingual, Spanish-English
  • Minimum one year experience working in a clinical environment
Preferred Qualifications:
  • Experience working with HIV-positive patients
  • Experience working in a Latino community/population or underserved populations
Physical Requirements:

Prolonged periods of screen time, sitting at a desk, and standing.
