Compensation: $25-$27 per hour
Employment Type: Part-Time, In-person, 7am-3pm
About the Organization: HCP Talent is collaborating with a Long-Term Care/Rehabilitation facility seeking a dedicated Social Worker (SW).
Key Responsibilities:
- Complete all Social Work documentation in alignment with facility policies and procedures.
- Coordinate interdisciplinary discharge planning while possessing knowledge of community resources.
- Ensure adherence to government and accrediting agency standards and regulations relevant to Social Work.
- Engage with facility departments and external agencies to develop a comprehensive care plan and facilitate necessary services.
- Act as a liaison between families and interdisciplinary team members.
