Lead Registered Nurse

Mass General Brigham is seeking a highly skilled and experienced Care Transition Specialist to lead our team in providing exceptional patient care and coordination. As a key member of our Post-Acute Capacity team, you will be responsible for facilitating seamless transitions between healthcare settings, ensuring patients receive the necessary resources and services to achieve optimal outcomes.
Key Responsibilities:- Coordinate referrals and placements to extended care facilities, home healthcare agencies, and other community resources.
- Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure continuity of care.
- Act as a liaison between healthcare providers, payers, and community organizations to resolve issues and ensure timely access to services.
- Monitor and evaluate the quality of care provided by extended care facilities and home healthcare agencies, making recommendations for improvement as needed.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.
- Bachelor's degree in a healthcare-related field and 3-5 years of experience in discharge planning, long-term care, or utilization review.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including patients, families, healthcare providers, and community organizations.
- Strong analytical and problem-solving skills, with the ability to navigate complex healthcare systems and resolve issues in a timely and effective manner.
- Proficiency in electronic medical records and other healthcare software, with the ability to learn new systems and technologies as needed.
