Care Coordination Specialist

Position Overview:

Department: Case Management

Salary: Grade 303 - $34.8274

Work Arrangement: ON-SITE

Key Responsibilities:

Program Support (85%):

Under general supervision, assist in managing routine referrals, assessments, and care transitions for all health plan members. Collaborate with healthcare networks to ensure timely access to members' clinical data. Work alongside Case Management personnel and other healthcare team members to facilitate medical services and resolve member inquiries efficiently. Conduct outreach to members to coordinate the completion of initial and annual health assessments. Review member concerns that may necessitate additional support or outreach with supervisory staff. Communicate initial care plans to primary care providers, health networks, and members as necessary. Facilitate referrals to mental health services and identify community resources to address member needs. Educate members on their Medi-Cal and CCS benefits and notify care teams of significant event triggers.

Administrative Support (10%):

Engage in a mission-driven culture focused on high-quality performance, emphasizing customer service, consistency, dignity, and accountability. Assist the team in fulfilling departmental responsibilities and collaborate with colleagues to support both short- and long-term departmental goals. Maintain accurate documentation of member cases, care plans, and interdisciplinary care teams within the management system.

Additional Duties (5%):

Complete other assigned projects and responsibilities as needed.

Minimum Qualifications:

High School diploma or equivalent is required. A minimum of one year of experience in a member service role addressing the needs of pediatric members is necessary. An equivalent combination of education and experience that enables successful performance of the essential duties may also be considered.

Preferred Qualifications:

Bachelor's degree in healthcare management, social work, human services, or a related field is preferred. Experience with Health Maintenance Organizations (HMOs), Medi-Cal, and pediatric health services is advantageous. Bilingual proficiency in English and another language (such as Arabic, Chinese, Farsi, Korean, Spanish, or Vietnamese) is preferred.
