As a key member of our interdisciplinary clinical team, you will provide high-quality, advanced acute care to patients in their homes, aligning with Mercy's mission, culture, and standards of practice.
Responsibilities- Deliver daily hospital-level patient care in the home setting, including clinical and home assessments, teaching, treatments, documentation, and coordination of care with HaH teams.
- Provide advanced acute skilled nursing care, ensuring patient safety and well-being.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Meet all state board of nursing requirements and maintain current RN licensure.
- Graduate of an accredited registered professional nursing program.
- A minimum of two years of previous nursing experience in an acute inpatient or emergency department setting.
- Basic Life Support Health Care Provider card or successful completion of course within 30 days.
- Strong nursing, communication, and organizational skills for home-based visit prioritization and efficiency.
- Ability to work with various computer software applications and technologies required.
