Job Summary

We are seeking a highly skilled Admissions and Care Coordinator to join our team at Tabitha. This role is responsible for working directly with clients, family members, hospital staff, physicians, and other community agencies to obtain referrals and provide navigation for seniors and residents.

Key Responsibilities
  • Coordinate the admissions process, ensuring completion and follow-up to provide appropriate services to seniors and residents.
  • Evaluate medical information to determine Medicare, Medicaid, and private insurance reimbursement issues, and act on identified issues.
  • Provide information to seniors and their families regarding financial concerns, including general information about Medicare, Medicare Part D, and application for Medicaid and other financial assistance programs.
  • Network and negotiate with community agencies and representatives of insurance companies and/or funding sources.
  • Complete daily management of bed availability for Tabitha's Living Communities to ensure the best possible patient outcomes and meet program goals and budgeted census.
Requirements
  • Bachelor's degree in human services or a related field, or graduate of an accredited school of social work or college of nursing (RN or LPN).
  • Minimum of four years of experience in gerontology, case management, or public health, with Medicare and Medicaid training/experience preferred.
  • Valid Nebraska driver's license and good driving record required.
  • Must possess auto insurance at or above Nebraska statutory requirements.
