About the RoleWe are seeking a highly skilled Oncology Social Worker to join our team. As a key member of our cancer care team, you will play a vital role in improving the health outcomes of our patients.
Key Responsibilities- Develop and implement a plan to improve coordination of cancer care for African-American and Hispanic patients in Lancaster County/Lincoln.
- Improve navigation for identified disparate populations and reduce health disparities in Lancaster County and the surrounding service area.
- Develop and manage a tool to track patients and provide staff education on cultural competence.
- Bachelor's degree in Nursing or Master's in Social Work required.
- Three years experience in Oncology care as an RN or Social Worker required.
- Excellent communication skills and verbal. Interpersonal and problem-solving skills necessary to interact with physicians, patients and their families as well as the greater community.
