The Admissions and Care Coordinator plays a vital role in ensuring the seamless transition of seniors into Tabitha Health Care SNF programs. This position requires strong communication and interpersonal skills to work effectively with clients, family members, hospital staff, physicians, and other community agencies.

The ideal candidate will have a strong understanding of gerontology and be able to navigate complex healthcare systems to ensure the best possible outcomes for our clients. Key responsibilities include:

  • Providing tours and general information related to the facility
  • Updating facility EMR and communicating with the Accounting department
  • Documenting and maintaining accurate senior/client records
  • Accessing and extracting information from Tabitha and hospital computer EMR systems
  • Providing weekend coverage and ongoing education to referring agents

The successful candidate will be a team player who is able to work effectively in a fast-paced environment and prioritize multiple tasks to meet deadlines. A strong understanding of Medicare and Medicaid regulations is preferred, as well as experience with computerized information systems.
