We are seeking a compassionate and skilled Hospice Supportive Care Nurse to join our team at Hospice Of Hope, Inc. As a key member of our care team, you will provide one-to-one care to terminally ill patients, focusing on their physical, emotional, and spiritual needs.
Responsibilities- Provide compassionate end-of-life care to patients and their families, adhering to individualized plans of care and demonstrating a commitment to timely assessment and response to patient needs.
- Educate patients and caregivers on end-of-life care, services, and resources, promoting informed decision-making and empowerment.
- Document complex medical information using clinical tools, data, and other relevant information to complete assessments of patient needs.
- Establish and maintain professional relationships with patients, families, and healthcare providers, utilizing strong written and verbal communication skills.
- Basic computer skills, including working knowledge of the Microsoft Office suite, are required for effective documentation and communication.
- Graduate of an accredited School of Nursing and current licensure, or eligibility to be licensed, in Kentucky and Ohio.
- At least one year of experience in a patient care position in a hospital, nursing home, or related healthcare facility is preferred.
- Personal qualities of empathy, responsibility, and dependability are essential for success in this role.
- Effective communication skills, both written and verbal, are necessary for collaboration with team members, patients, providers, and family members.
- The ability to work independently, with minimal direct supervision, is required.
- Valid driver's license, current auto insurance, and reliable transportation are necessary for travel to client homes, nursing homes, hospitals, and other facilities.
