Physical Therapist

Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility, is seeking a dedicated Physical Therapist to join our team. As a bridge between hospital and home, we provide luxurious and hotel-like environment for those in need of Post Acute Rehabilitation or Long-Term Care.
Responsibilities- Assist in planning, organizing, developing, and implementing Physical Therapy services to ensure the highest degree of quality resident care.
- Develop and implement individualized therapy plans to help residents reach their highest level of functioning.
- Interpret and instruct in physical therapy policies and procedures to personnel, residents, and family members as necessary.
- Participate in discharge planning, development, and implementation of resident care plans, resident assessments, and other necessary tasks.
- Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
- Maintain treatment records, resident files, and progress notes as required.
- Current license and registration to practice as a Physical Therapist in the State of NY.
- Current malpractice insurance and license in good standing.
- Dedicated to providing extraordinary compassionate care.

- Excellent communication, customer service, and organization skills.
- Trusted to work independently and in a team environment.
- Able to work well under pressure, flexible, and will work harmoniously with all staff.
