Nurse Practitioner or Physician Assistant-Certified

The Nurse Practitioner or Physician Assistant Certified (Advanced Practice Provider-APP) is responsible for providing comprehensive family medicine services to patients of Columbia Valley Community Health. This role involves providing primary care services in a patient-centered medical home, including non-clinic sites such as nursing homes, patient homes, school-based clinics, mobile clinics, and tele-health settings.
Key Responsibilities- Provides comprehensive and continuous primary care services to patients.
- Consults with clinic physicians, specialists, pharmacists, nurses, and other healthcare professionals as needed.
- Assesses patient needs for specialty referrals, diagnostic and in-patient care, and makes appropriate referrals.
- Participates in the development and utilization of CVCH practice guidelines and procedures.
- Responsible for accurate, timely, and complete documentation in patient records.
- Responsible for accurate and timely completion of billing documents.
- Performs minor outpatient procedures in accordance with CVCH privileges.
- Provides patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and follow-up as appropriate.
- Provides clinical supervision and training of ancillary healthcare staff to ensure quality of care for CVCH patients.
- Responsible for maintenance of credentials essential for practice, including licensure, certification, and CME.
- Attends all required staff meetings, provider meetings, and participates in CVCH committees as directed by Medical Director.
- Graduate from an accredited Nurse Practitioner Program or Graduate from an accredited Physician Assistant Program.
- Certification as a Physician Assistant.
- A valid license to practice as an ARNP with Evidence of Prescriptive Authority in the State of Washington OR PA-C in Washington State or eligible to obtain licensure.
- Basic Life Support (BLS) required. ACLS or ATLS may be used to substitute for BLS.
- Three (3) years of experience working in a community health center or managed care environment preferred.
- Bilingual in English/Spanish preferred.
