The Community Paramedic Specialist delivers advanced pre-hospital care, adhering to the Pennsylvania Emergency Medical Services Act and associated regulations. This role serves individuals across all age groups, ensuring high-quality healthcare delivery.

In collaboration with other healthcare professionals, the specialist will engage in social support initiatives within the patient's residence or community, enhancing overall health outcomes.


Key Responsibilities:


Operating within the defined scope of practice as per the Pennsylvania Emergency Medical Services Act, this position focuses on providing healthcare in non-hospital settings. The aim is to elevate patient health through comprehensive assessments and treatments, coordinated with the Advanced Medical Home, care managers, primary care physicians, and other healthcare providers.

Responsibilities include:

  • Maintaining clinical skills through ongoing education and training.
  • Communicating educational needs to the Mobile Integrated Healthcare Coordinator and other leadership.
  • Ensuring compliance with Medical Command Authorization and other necessary credentials.
  • Conducting vehicle and equipment inspections at the start of each shift and after patient interactions.
  • Performing routine medication checks and maintaining accurate documentation.
  • Providing mentorship to new staff and students from affiliated programs.
  • Actively participating in patient safety initiatives and identifying potential risks.
  • Engaging in required safety education programs and reporting incidents per established protocols.
  • Developing patient evaluation plans to ensure the accuracy of health information.

The Community Paramedic Specialist will address identified health needs, conduct home safety evaluations, and contribute to improving the quality of life for patients.

This role encompasses outreach, wellness initiatives, health screenings, immunizations, and disease management, with a focus on chronic conditions such as diabetes and heart failure. Additionally, the specialist will recognize mental health concerns and facilitate referrals to appropriate services.

Wound care and participation as integral members of a healthcare team are also key components of this position. Work is typically conducted in a clinical environment, with accountability for meeting job-specific responsibilities and adhering to organizational policies.


Position Details:
Hours: 8 AM to 6 PM (Rotating Weekends)
Experience: Minimum of 5 years in a related field and possession of a paramedic certification is essential.

This position is dedicated to delivering personalized care to medically complex patients in their homes.

Education: Graduation from a Specialty Training Program is required.

Experience: A minimum of 5 years of relevant work experience is required.

Certification(s) and License(s): Certified Paramedic - Default Issuing Body.
