The Community Mobile Paramedic delivers advanced pre-hospital medical care, adhering to the Pennsylvania Emergency Medical Services Act and associated regulations. This role is pivotal in enhancing the health of individuals across various age groups through integrated healthcare services within their homes or communities.
Key Responsibilities:
- Provide comprehensive healthcare in out-of-hospital settings, focusing on patient assessment and treatment in collaboration with the Advanced Medical Home, care managers, and other healthcare professionals.
- Maintain and enhance clinical skills through continuous education and practical experience.
- Communicate effectively with the Mobile Integrated Healthcare Coordinator and other relevant authorities regarding educational needs.
- Ensure compliance with Medical Command Authorization and maintain necessary credentials.
- Participate in ongoing education programs relevant to the role.
- Conduct thorough inspections of vehicles and equipment, ensuring readiness for patient encounters.
- Perform regular medication audits and maintain accurate documentation.
- Provide mentorship to new staff and students from affiliated programs.
- Engage actively in patient safety initiatives and identify potential risks to improve healthcare delivery.
- Adhere to organizational policies and procedures while fulfilling job responsibilities.
- Develop patient evaluation strategies to ensure accurate and comprehensive information collection.
- Conduct home safety assessments and respond to health needs, ultimately enhancing patient quality of life.
- Facilitate outreach and wellness initiatives, including health screenings, immunizations, and disease management.
- Recognize mental health concerns and facilitate referrals to appropriate services.
- Provide wound care and participate as an integral member of the healthcare team.
Position Details:
- Hours: 8 AM to 6 PM (Rotation of Weekends)
- 5 years of experience and paramedic certification are mandatory.
- Local travel is essential to provide patient care.
- This role focuses on delivering personalized care to complex health plan members in their residences.
