Role Overview:

Memorial Health University Medical Center is seeking a skilled Advanced Practice Provider to play a key role in our multidisciplinary team. The successful candidate will have the opportunity to make a meaningful impact on patient outcomes and experiences while working collaboratively with healthcare professionals.

Key Accountabilities:

  • Develop and implement comprehensive care plans that address the unique needs of patients
  • Conduct thorough assessments and evaluations to identify areas for improvement
  • Collaborate with healthcare teams to develop and implement quality improvement initiatives
  • Provide education and support to patients and families to promote health literacy and self-management
  • Participate in ongoing quality improvement and professional development initiatives

Requirements:

  1. Certification as a Nurse Practitioner (FNP) is required; certification in a relevant specialty may be preferred
  2. Maintain current licensure and certification as required by Illinois State law
  3. A minimum of 2 years post-graduate school experience as an APN is required; prior leadership experience is highly valued
  4. Valid CPR certification is required; ACLS certification may be preferred
  5. Maintain DEA licensure as required by federal and state regulations

Benefits and Compensation:

The estimated annual salary range for this position is $95,000 - $145,000, commensurate with experience and qualifications.
