Comprehensive Care Coordinator

As an Advanced Practice Provider at Memorial Health University Medical Center, you will play a vital role in delivering exceptional patient care and advancing our mission.
Key Responsibilities:- Foster strong relationships with patients, families, and healthcare teams to provide coordinated care.
- Develop and implement effective treatment plans, incorporating evidence-based practices and patient preferences.
- Participate in interdisciplinary collaborations to improve patient outcomes and quality of care.
- Certified Nurse Practitioner (CNP) or Family Nurse Practitioner (FNP), as determined by the department.
- Illinois State licensure for Advanced Practice Nurses.
- A minimum of 2 years of post-graduate experience as an APN.
- Current CPR and ACLS certifications.
- DEA licensure where required.
The estimated annual salary for this position is $125,000 - $155,000, based on industry standards and location.
