About the Job

We are seeking a highly skilled Healthcare Professional to join our team as a Physician - CenterWell - Wesley Chapel. As a key member of our integrated care teams, you will provide compassionate, personalized care to patients, addressing their physical, emotional, and social wellness.

This role requires a strong understanding of Medicare guidelines and coverage, as well as experience managing Medicare Advantage panels of patients in a coordinated care environment. The ideal candidate will possess excellent verbal and written communication skills, with the ability to work effectively in a value-based care setting.

The successful candidate will be responsible for evaluating and treating center patients in accordance with standards of care, following level of medical care and quality for patients, and monitoring care using available data and chart reviews. They will also assist in the coordination of patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.

Key Responsibilities:

Required Qualifications:

  • Current and unrestricted medical license or willingness to obtain a medical licenses in state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
  • Graduate of accredited MD or DO program of accredited university.
  • Excellent verbal and written communication skills.
  • Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
