About the Role

The Population Health Registered Nurse serves as a key member of the Galileo team, playing a vital role in improving patient outcomes and quality of care. As a Population Health Registered Nurse, you will be responsible for providing telephonic outreach to patients, conducting post-discharge assessments, and educating patients on preventative health screenings and chronic disease management.

You will work closely with patients to identify barriers to medication adherence, safety concerns, and social needs, and develop personalized plans to address these issues. Your expertise in population health and case management will enable you to navigate complex healthcare systems and coordinate care between healthcare services, pharmacies, and community providers.

This is a remote, full-time position that requires a high level of autonomy and flexibility. You will be working with a dynamic team that is passionate about improving healthcare outcomes and quality of care.

Responsibilities
  • Conduct telephonic outreach to patients for post-discharge assessments and education
  • Develop and implement personalized plans to address patient barriers to medication adherence, safety concerns, and social needs
  • Coordinate care between healthcare services, pharmacies, and community providers
  • Provide education and support to patients on preventative health screenings and chronic disease management
  • Collaborate with internal teams to improve population outcomes and quality of care
Requirements
  • 2+ years of experience in population health, case management, or primary care with Medicare and Medicaid populations with complex chronic conditions
  • Nurse Licensure Compact (NLC) required
  • Highly culturally competent with diverse populations
  • Comfortable with using technology and various applications to deliver care to patients virtually
  • Excellent written and verbal communication skills
  • Ability to thrive in a flexible start-up environment
