Position Title: Housing Care Manager
ROLE SUMMARY
The Housing Care Manager plays a vital role within the St. John's Community Health Housing Program team, collaborating with Medi-Cal members to deliver essential homeless and housing support services. This position focuses on providing comprehensive Housing Navigation services, which encompass client advocacy, case management, and connecting individuals to mental health and substance abuse resources, as well as stable housing solutions.
Key Responsibilities:
- Conducting thorough Housing Assessments to identify client preferences and obstacles related to housing.
- Creating tailored Individualized Housing Support Plans that address specific barriers and outline both short- and long-term housing objectives.
- Facilitating the housing application process and assisting clients in gathering necessary documentation.
- Advocating for benefits and supporting clients through the SSI application process.
- Identifying and securing resources to help subsidize rent and matching clients with available rental assistance programs.
- Communicating effectively with landlords on behalf of clients to ensure successful housing retention.
- Providing ongoing support and education to clients regarding their rights and responsibilities as tenants.
- Offering guidance on maintaining positive relationships with landlords and property managers.
- Assisting clients with lease compliance and addressing any issues that may threaten their housing stability.
Qualifications:
- A Bachelor’s degree or higher is required.
- Knowledge of housing resources, including Veteran Services and Rapid Rehousing.
- Experience in social services or public health, with a minimum of 2 years in the field.
- Ability to work with diverse populations, including those experiencing homelessness and individuals with mental health or substance use challenges.
- Valid driver's license and access to a vehicle for travel to various service locations.
- Bilingual proficiency in English and Spanish is strongly preferred.
