Job Summary:

This Physical Therapist position supports our Outpatient Rehabilitation services, working primarily in multiple locations including Indian Trail, Matthews, and Union/Monroe. The successful candidate will evaluate patients referred by physicians, planning and conducting programs to restore function, prevent disability, and promote independence.

Duties and Responsibilities:

  • Evaluates patients and develops tailored care plans incorporating modalities, procedures, exercise programs, and patient education based on diagnosis and condition.
  • Gathers and interprets relevant data from medical records, physician reports, and patient interactions, identifying assistive device needs.
  • Documents evaluations, tests, progress notes, daily treatment logs, attendance records, patient education, outcomes, discharge notes, and charges.
  • Re-evaluates patients as necessary, modifying goals and treatment based on changes in patient status.
  • Communicates evaluation results and progress to physicians, referral sources, and clinical staff, coordinating schedules, treatment programs, and recommending solutions to patient care issues.
  • Provides individualized instruction for home programs, including equipment use, and documents patient/family education regarding deficits, progress toward goals, and therapeutic techniques.
  • Directs and supervises activities of Physical Therapist Assistants, Technicians, students, and volunteers, assisting with new staff orientation and training.

Work Environment:

The ideal candidate will thrive in a fast-paced, dynamic clinical environment. This role requires standing, walking, and lifting up to 50 pounds occasionally, with the ability to demonstrate exercises and activities to patients/caregivers. Personal Protective Equipment may be required due to potential exposure to hazardous substances or bodily fluids.
