We are seeking an experienced MDS Nurse to join our team at Royalton Manor. As a key member of our care management team, you will be responsible for conducting assessments and care plan coordination for residents assigned to you.
Key Responsibilities- Complete the MDS, CAA's and care plans within regulated time frames.
- Assess residents through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
- Review information from hospital, consults, and outside agencies, and use such information in the completion of the assessment and care planning.
- Coordinate, identify, and/or initiate significant change MDS.
- Prepare to conduct PPS meetings, maintaining MDS assessments per Medicare schedule, and maintain PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
- Remain current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.
- Active Registered Nurse (RN) license with the state of Michigan.
- AANC certification a plus. RAC-CT.
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred.
- Experience as an MDS Nurse.
