Job Summary

We are seeking a highly motivated and experienced Activity Coordinator to join our team at Pace of Southwest Michigan. As an Activity Coordinator, you will be responsible for planning and coordinating therapeutic and recreational activities for our participants, promoting their physical, mental, and social well-being.

Key Responsibilities
  • Conduct initial assessments and scheduled reassessments of all participants to understand their needs and preferences.
  • Develop and implement specific therapeutic programs that cater to the cognitive and functional continuum of our participants.
  • Plan and provide group and individual activities, including community outings and special events, to promote engagement and socialization.
  • Communicate with placement facilities to assess participant activity and involvement and work with facility staff to ensure seamless care.
  • Create monthly and daily calendars to inform participants and staff of daily activities.
  • Participate as a member of the Interdisciplinary Team (IDT) to ensure comprehensive care and support for our participants.
  • Coordinate with various community resources to provide additional support and services to our participants.
  • Perform home visits as directed to ensure continuity of care.
  • Abide by all established Pace of Southwest Michigan policies, rules, and regulations.
Requirements
  • High school diploma or equivalent required.
  • Minimum of one year of experience in social or recreational programs providing and coordinating services for the frail and elderly population.
  • Valid MI Driver's License preferred; possess current CPR certification by AHA or willing and able to obtain as offered by Pace of Southwest Michigan.
  • Ability to work a flexible schedule and travel in all kinds of weather.

The employee frequently is required to sit and use hands and fingers, handle or feel objects, tools, or controls. The employee is occasionally required to stand and walk. The employee must be able to travel in all kinds of weather.

While performing the duties of this job, the employee is required to frequently move up to 50 pounds and perform physical demands expected while providing medical care for an elderly population.
