Job Summary

Best Care Home Health is seeking a skilled and compassionate Home Health Nurse to join our team. As a Home Health Nurse, you will provide high-quality patient care and coordinate services to meet the unique needs of our patients.

Key Responsibilities
  • Conduct comprehensive health and psychosocial evaluations to identify patient needs and develop individualized care plans.
  • Provide direct nursing care, including administering medications and treatments as prescribed by physicians.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Monitor patient progress, update care plans, and make necessary revisions to ensure optimal outcomes.
  • Supervise and educate home health aides and LPNs to ensure high-quality patient care.
  • Maintain accurate and detailed clinical records, including progress notes and updates to primary physicians.
Requirements
  • Graduate of an accredited school of nursing with a current licensure in the state.
  • At least one year of recent experience working as a professional nurse in an acute care setting.
  • CPR certification and valid driver's license.
Preferred Qualifications
  • Associates or Bachelor's degree in nursing.
  • Experience working in home health care or a related field.
