As a Nursing Care Coordinator, you will play a vital role in ensuring the highest quality of care for our residents. Your responsibilities will include:
- Participating in resident care coordination to ensure seamless transitions and effective communication.
- Accurately documenting resident condition and nursing needs to inform care decisions.
- Monitoring and adjusting nursing assistant assignments to optimize care delivery.
- Administering medications and treatments as prescribed by physicians.
- Initiating physician orders to ensure timely and effective care.
- Coordinating admissions, discharges, and transfers to ensure a smooth resident experience.
Requirements:
- Graduate from an accredited nursing program.
- Active Practical Nurse license.
