Nurse Care Coordinator

Key Responsibilities:
- Participate in interdisciplinary team reviews to plan and coordinate resident care.
- Document resident conditions and nursing needs accurately and in a timely manner.
- Develop and direct nursing assistant assignments based on census and level of care required.
- Administer medications and document reactions.
- Initiate physician orders and facilitate communication with families.
- Establish protocols for reviewing and updating care plans.
- Coordinate admissions, discharges, and transfers to deliver quality customer service.
Requirements:
- Licensed Practical Nurse graduate from an accredited nursing program.
- Certifications and licensure required by state regulations.
