About the Role

We are seeking a Homebound Client Manager to join our team and manage a caseload of senior homebound clients. The successful candidate will have a strong background in geriatric care and the ability to work effectively with a diverse client population.

Responsibilities:

  1. Conduct written, in-home and phone assessments of clients
  2. Develop care plans and coordinate services
  3. Provide resources and referrals for clients as needed
  4. Report all suspicions of elder abuse or neglect immediately to supervisor

Requirements:

  • Bachelor's degree in Gerontology or equivalent experience
  • Two years experience in a geriatric care setting
  • Bi-lingual (Spanish speaking, writing) candidate preferred
  • Background Fingerprint Clearance (Live SCAN)
  • Drug Test Clearance
