Homeless Services Advocate Lead

About the Role:
SELF Inc. is seeking a highly skilled Homeless Services Advocate Lead to join our team. As a key member of our organization, you will play a crucial role in providing trauma-informed case management support to participants in various emergency housing, Safe Haven, and supportive housing programs.
Job Summary:
The Homeless Services Advocate Lead will provide direct service links to external resources necessary to establish safe, permanent housing and supports to end the participant's homelessness. This position requires strong interpersonal, communication, and people skills to work effectively with a diverse population of individuals.
Key Responsibilities:
- Caseload Management: Provide case management services to a caseload of 25-35 participants depending on the assigned program location.
- Assessment and Planning: Perform an initial assessment of each participant assigned to your caseload within 72 business hours of their enrollment into the program.
- Resource Development: Identify, screen, and develop plans to connect participants needing alcohol, drug, mental health, or medical services to appropriate providers.
- Documentation and Reporting: Maintain accurate and complete documentation of case management services, interaction, and participant progress.
- Collaboration and Communication: Participate in Exchange Meetings, Participant Progress Reviews, and collaborative meetings with external agencies working with program participants.
Requirements:
- Education: Bachelor's Degree in a human services field, including but not limited to Social Work from an accredited University or College.
- Experience: Minimum of three years of acceptable training and experience in homeless, mental health, and/or addiction treatment services.
- Skillset: Strong verbal and written communication skills, crisis intervention, conflict resolution, group facilitation, and counseling.
