Clinical Nurse Specialist

The Zufall Health Center is seeking a highly skilled Registered Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and case management services to our patients.
Key Responsibilities- Provide face-to-face nursing assessment and reassessment to include history taking and an appraisal of the patient's health status and needs.
- Develop and implement individualized nursing plans to meet the unique needs of each patient.
- Work with patients to provide education and support on disease management, risk reduction, and harm reduction.
- Coordinate care with other healthcare providers and community resources to ensure timely and effective care.
- Assist patients with accessing and maintaining free pharmaceuticals through the Patient Assistance Program.
- Provide system navigation and support to patients and families, addressing concerns and initiating corrective action as needed.
- Registered Nurse license in the State of New Jersey.
- One to two years of experience in a clinical setting, including direct patient care and triage experience.
- Prior experience in an ambulatory care or outpatient clinic setting and case management experience preferred.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- A competitive salary range of $76,000.00 to $83,910.14.
