Jet Health is seeking a highly skilled Home Health Occupational Therapist to provide exceptional care to patients in their homes.

Job Description

We are looking for a compassionate and dedicated Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will be responsible for providing one-on-one patient care, conducting ongoing clinical assessments, and developing treatment plans tailored to each patient's needs.

  • Main Responsibilities:
  • Conduct thorough assessments to identify patient needs and determine levels of functioning
  • Evaluate patients' occupational performance in areas such as self-care, work, leisure, and rest
  • Develop and implement direct therapeutic interventions to address ROM, strength, coordination, adaptive equipment needs, and environmental modifications
  • Collaborate with patients, representatives, and caregivers to develop and revise treatment plans
  • Document clinical notes, phone orders, and other records in a timely manner
Requirements

To be successful in this role, you must have:

  • Education: Bachelor of Science in Occupational Therapy
  • Licensure: Current State License as an Occupational Therapist and a valid driver's license
  • Experience: One year of experience as an Occupational Therapist, with home health experience preferred
  • Skills: Proficiency in therapy skills, good interpersonal skills, current CPR certification, and knowledge of durable medical equipment
