Position Overview:

Under the guidance of the Director of Ambulatory Care, the Clinical Operations Manager plays a pivotal role in enhancing the functionality of San Ysidro Health's Patient-Centered Medical Home (PCMH) Care Teams. This position is essential in collaboration with medical leadership to ensure the effective execution of the San Ysidro Health Care Model, aimed at delivering safe, high-quality healthcare and improving health outcomes by addressing the population health management needs of the communities served.

The Clinical Operations Manager directly oversees the Ambulatory Care Supervisors (RN) and guarantees that adequate training and resource support are available for the provision of licensed nursing and non-licensed clinical care across designated outpatient facilities. This role identifies specific opportunities to enhance health potential, improve health outcomes, and address care gaps by analyzing provider, care team, and clinic performance metrics. The Clinical Operations Manager is empowered, in conjunction with other leaders, to refine the San Ysidro Health Care Model and service offerings to cater to the diverse populations represented.

Key Responsibilities:
  • Collaborate with Clinic Leads to facilitate coordinated care in alignment with the patient-centered SY Health model, ensuring adherence to patient safety and satisfaction, population health outcomes, and clinical quality objectives; guarantee that staff delivers the highest standard of care in compliance with the Nurse Practice Act, medical assistant scope, NCQA, TJC, and all relevant regulations.
  • Work in partnership with all care team members to address the healthcare needs of assigned populations through continuous data monitoring, trend identification, care team training, and the implementation of workflow efficiencies, pilots, and initiatives.
  • Utilize sound process design principles to develop, implement, and oversee departmental protocols, policies, and procedures that promote patient safety and satisfaction, effective care delivery, and the achievement of organizational goals.
  • Assist in the formulation of clinic staffing standards and plans, clearly defining the roles and responsibilities of the Care Team Supervisor and other assigned positions.
  • Select, train, supervise, and mentor Care Team Supervisors capable of managing assigned care team personnel, overseeing care team operations, and cultivating teams that meet the evolving needs of the communities served.
  • Coordinate the annual continuing education schedule for assigned sites, ensuring the clinical competency of each nurse and medical assistant prior to the assignment of independent care activities, and validate clinical competency annually and as required.
  • Analyze health data and standard clinical performance metrics (UDS, HEDIS, and Utilization); compile and disseminate reports to effectively manage the populations of assigned sites and inform the care team's daily, weekly, and monthly activities. Utilize this analysis to enhance the San Ysidro Health Care Model through pilot programs and initiative development.
  • Establish consistent communication channels and regularly convene with care teams to identify and implement strategies for improving population management outcomes.
  • Leverage the strengths of the San Ysidro Health spectrum of care to facilitate patient access to services, enhancing health outcomes and supporting patient navigation through the external healthcare system.
  • Maintain professional licensure and continuously expand professional knowledge to include standards of clinical practice, regulatory changes, and industry innovations that empower care teams to meet established objectives.
Additional Responsibilities:
  • Assist in the development and implementation of policies and procedures.
  • Train staff at various levels.
  • Represent San Ysidro Health at meetings and conferences.
  • Utilize and organize time efficiently to maximize productivity.
  • Work both independently and collaboratively in a team-oriented environment.
  • Adapt to shifting priorities, demands, and timelines through analytical and problem-solving skills.
  • Travel to multiple sites within the San Ysidro Health network as required.
  • Perform other duties as assigned.
Qualifications:

Experience Required:
  • A minimum of 5 years of clinical Registered Nursing experience with oversight of direct reports and/or unit management.
Experience Preferred:
  • Experience managing teams of clinicians and collaborating with various departments and other managers.
  • A solid understanding of financial and budgetary processes within an organization, including cost analysis and metric improvement.
  • Proven experience in reducing turnover, enhancing quality metrics, developing staff training materials, and leading training initiatives.
Education Required:
  • Graduate of an accredited registered nursing program.
  • Associate Degree in Nursing (ADN).
Education Preferred:
  • Bachelor's or Master's degree in nursing or a healthcare-related field.
Certifications Required:
  • Current California Registered Nurse (RN) license.
  • Valid driver's license with appropriate insurance coverage.
Communication Skills Required:
  • Exceptional written and verbal communication skills.
  • Strong interpersonal skills.
Technical Skills Required:
  • Demonstrated high-level understanding of the PCMH model of care.
  • Strong problem-solving abilities in a timely manner.
  • Ability to work efficiently and effectively with medical providers and support staff.
  • Comfortable using computer-based tools, processes, and workflows in job performance.
Equipment Used:

Working Conditions:
  • May involve long and sometimes irregular hours, including weekends.
  • Ability to sit and stand for extended periods.

Universal Requirements:
