Job Summary

We are seeking an experienced Registered Nurse to serve as a Patient Care Manager in our Home Health team. This role requires strong clinical expertise, excellent leadership skills, and the ability to work collaboratively with patients, families, and healthcare professionals.

Responsibilities

The successful candidate will be responsible for:

  • Collaborating with healthcare teams to coordinate patient care and services
  • Evaluating patient outcomes and identifying opportunities for improvement
Requirements

To be considered for this role, you must possess:

  • A current RN licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
