Job Responsibilities
Nurse Care Manager Role
We seek an experienced healthcare professional to fill the Nurse Care Manager position at Curant Health in Smyrna, GA. As part of our team, you will be responsible for managing care plans for patients with chronic conditions. Key duties include:
  • Evaluating patients' current health status and treatment plans to identify opportunities for improvement.
  • Designing and implementing individualized care plans to promote optimal health outcomes.
  • Communicating effectively with patients, families, and healthcare providers to ensure seamless care coordination.

Key Qualifications
To succeed in this role, you must hold a Bachelor's degree in Nursing, have a minimum of 2 years of clinical nursing experience, and be licensed as a LPN or RN. Additionally, you should possess strong analytical and problem-solving skills, as well as the ability to work collaboratively within a multidisciplinary team.
