Care Coordination for Medically Fragile Patients
Aveanna Healthcare is hiring a skilled Nurse to coordinate care for a medically fragile patient in the school setting. As a School Case RN, you will work closely with the patient's healthcare team to ensure seamless care transitions and communicate effectively with all stakeholders. You will also conduct regular patient assessments, document care provided, and collaborate with other healthcare professionals to achieve optimal patient outcomes.

About the Job
In this position, you will use your exceptional communication skills to build strong relationships with the patient, their family, and healthcare providers. You will also utilize your clinical expertise to develop and implement individualized care plans that address the patient's unique needs and goals. Additionally, you will provide emotional support and guidance to patients and families when needed.

Responsibilities
- Coordinate care transitions for patients
- Conduct regular patient assessments and document care provided
- Communicate effectively with healthcare providers and stakeholders
- Collaborate with other healthcare professionals to achieve optimal patient outcomes

Qualifications
- Must have and maintain an active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
- Current CPR certification from American Red Cross or American Heart Association
- TB skin test (current within last 12 months)

Estimated Salary: $42.00 per hour
