We are seeking a highly skilled LPN Medication Care Manager to join our team at Sunrise Senior Living. As a Medication Care Professional, you will play a vital role in ensuring the highest quality of care for our residents by administering medications and treatments in a safe and organized manner.
Key Responsibilities- Medication Administration: Administer medications and treatments to residents in accordance with their individualized care plans and Sunrise Senior Living policies.
- Documentation and Record-Keeping: Accurately document and maintain records of medication administration, including resident medication profiles and medication pass records.
- Collaboration and Communication: Work closely with the Resident Care Director, Wellness Nurse, and other healthcare professionals to ensure seamless communication and coordination of care.
- Risk Management and Safety: Identify and report any potential safety risks or hazards, and take proactive steps to mitigate them.
- Training and Development: Participate in ongoing training and education to stay up-to-date on the latest medication administration practices and Sunrise Senior Living policies.
- LPN License: Must be a licensed LPN or LVN in good standing.
- Certifications: Must maintain current certifications in CPR and First Aid.
- Experience: Previous experience working with seniors and in a medication administration role is preferred.
- Skills: Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
