Care Manager

We are seeking a dedicated and compassionate Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing exceptional care and services to our residents and their families.
Responsibilities- Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
- Develop and implement Individualized Service Plans (ISPs) to meet the unique needs and preferences of each resident.
- Participate in the development of care plans and collaborate with the community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies.
- Respond to the dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
- Encourage and assist residents in life skills and other life-enriching activities, as indicated on their ISP and demographic profile.
- Dedication to serving seniors with excellent customer service skills.
- Positive attitude, flexibility, and ability to work in a team environment.
- High School diploma/GED accepted and may be required per state regulations.
- Previous experience working with seniors preferred.
- Ability to make choices, decisions, and act in the resident's best interest.
- Competent in organizational and time management skills.
- Demonstrate good judgment, problem-solving, and decision-making skills.
