We are seeking a skilled and compassionate LPN to join our team of dedicated healthcare professionals at West Virginia University Health System.

About the Job

As an LPN Home Care Nurse, you will provide direct and indirect nursing care to patients in their homes under the supervision of a Registered Nurse (RN). You will work collaboratively with patients, families, and healthcare teams to implement and evaluate nursing care according to established systems and functions.

Key Responsibilities
  • Perform and monitor basic patient care according to established policies and procedures.
  • Use the nursing process to meet patients' physical and psychological needs.
  • Provide a safe and comfortable environment in patients' homes.
  • Ensure patient privacy and dignity.
  • Communicate patient care problems or potential problems to the primary RN and administrative staff.
  • Maintain patient confidentiality.
  • Function effectively as a LPN in terms of organization, communication, and prioritization.
Minimum Qualifications
  • Graduate of an LPN school.
  • Current unencumbered licensure with the state board of practical nursing.
Preferred Qualifications
  • One year of home care nursing experience.
Core Duties and Responsibilities
  • Perform and monitor patient teaching according to policy and procedures.
  • Anticipate, modify, and adapt teaching plans to accommodate patients and families.
  • Evaluate the effectiveness of teaching plans on a regular basis.
  • Coordinate care to ensure medications are administered as ordered by physicians.
  • Report findings of discrepancies to the primary RN and administrative staff.
Technical Skills and Duties
  • IV therapy, including ports, PICCs, TLC catheters, and sterile dressing changes.
  • Wound vacs, sterile wound care, Pleurx drain care, ostomy care, trach care, and Foley and suprapubic caths.
  • Labs via peripheral and central access.
  • Revolution device changes.
  • Complete head-to-toe assessments.
Physical Requirements
  • Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling.
  • Lifting, moving, and positioning patients of all weights.
  • Hearing within normal range.
  • Visual acuity within normal range.
Working Environment
  • Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases.
  • Exposure to toxic gases, fumes, and odors.
  • Exposure to high stress and constant interruptions.
Skills and Abilities
  • Demonstrate competency in technical and clinical skills.
  • Administer medications by all acceptable routes as ordered by physicians.
  • Communicate plan of care and patient needs effectively.
  • Anticipate teaching needs of each patient.
  • Make sound, independent judgments based on scientific principles.
