Job Title: Home Health Nurse

We are seeking a skilled and compassionate Home Health Nurse to join our team at WVUHS Home Care, LLC. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:
  • Provide direct and indirect nursing care to patients in their homes, including assessments, treatments, and education.
  • Collaborate with patients, families, and healthcare teams to develop and implement care plans that meet individual needs.
  • Conduct thorough assessments of patients' physical and psychosocial needs, identifying areas for improvement and developing strategies for care.
  • Administer medications and treatments as ordered by physicians, ensuring accurate and timely delivery.
  • Document patient progress and care provided, maintaining accurate and detailed records.
  • Communicate effectively with patients, families, and healthcare teams, ensuring seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as a Registered Nurse in the state of West Virginia or neighboring state.
  • Minimum one year of experience in home health care or a related field.
  • Ability to work independently and as part of a team, with strong communication and interpersonal skills.
  • Strong assessment and problem-solving skills, with ability to prioritize tasks and manage time effectively.
  • Proficiency in electronic medical records and other relevant software systems.
Working Environment:
