Job Summary

We are seeking a highly skilled and compassionate Self-Direction Coordinator to join our team at Economic Opportunity Council of Suffolk Inc. The ideal candidate will possess excellent communication skills, a strong ability to work independently, and a passion for supporting individuals with intellectual disabilities.

Key Responsibilities
  • Assist individuals and families in developing and maintaining their self-directed services budgets
  • Review and update budgets, Personal Care Plans, and Habilitation Plans to ensure effective service delivery
  • Recruit and hire staff to support individuals in their self-directed services
  • Provide assistance with expenditures and ensure timely payment of approved goods and services
  • Collaborate with individuals, brokers, and OPWDD to facilitate the self-direction process
  • Respond to emails and voice mails in a timely manner
  • Serve as a liaison between individuals, brokers, and OPWDD to ensure seamless service delivery
  • Monitor budget utilization and ensure services are aligned with individual outcomes
  • Provide recruitment and payroll support to self-hired staff
  • Schedule launch and team meetings as required
  • Complete assignments in a timely manner and prepare statistical data
  • Attend ISP meetings and update DDP2 as necessary
  • Review Habilitation Plans and Safeguards every 6 months or as required
  • Review and correlate staff attendance, time-sheets, and service documentation
  • Ensure services are related to listed outcomes and monitor quality of services delivered
  • Stay up-to-date with changes in the field and perform other related duties as requested
Requirements
  • Ability to follow instructions and respond to management's directions accurately and efficiently
  • Demonstrates accuracy and thoroughness, monitoring own work to ensure quality is met
  • Exceptional communication skills and ability to work independently with minimal supervision
  • Ability to prioritize work activities and use time efficiently
  • Maintain confidentiality and promote a positive team-oriented environment
  • Ability to stay focused and concentrate under normal or heavy distractions
  • Maintain a professional and clean appearance at all times consistent with company standards
  • Valid NY State Driver's License and Insured Vehicle with 100/300 Bodily Injury Liability Insurance
  • PPD Test (TB)
Preferred Qualifications
  • BA and experience working in the OPWDD field
  • Bi-lingual (Spanish) preferred
