We are seeking a highly skilled and compassionate Social Worker to join our Mental Health Clinic team in Central Islip. As a key member of our team, you will provide evaluations, treatment planning, psychotherapy, and crisis intervention to children, families, and adults with a full range of DSM diagnoses.
Responsibilities- Provide individual and group therapy to adults and children with a full range of DSM diagnoses in a clinic setting.
- Direct service will be provided both in person and via telehealth.
- Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record, including progress notes, comprehensive assessments, and individual action plans.
- Provide services that are delivered in a trauma-informed, culturally and linguistically competent manner.
- Communicate with all relevant referral sources, care coordination providers, and family members.
- Coordinate and provide linkage to community resources and for discharge planning.
- Attend routine supervision and team meetings.
- Provide crisis intervention during scheduled work hours as needed.
- Identify and recommend additional referrals as needed.
- Master's degree in social work required.
- LMSW, LMHC, LCSW, or limited permit required.
- At least 1 year of experience working in a behavioral health setting required; clinic experience required.
- Prior experience diagnosing using the DSM V and experience with behavioral/therapeutic interventions for children, adolescents, and adults preferred.
- Excellent verbal and written communication skills required.
- Proficient computer skills, including Microsoft Office and Electronic Health Records, required.
- Bilingual in Spanish preferred.
- Valid and clean NYS Driver's License required.
- Must be able to work a flexible schedule.
