Advanced Practice Nurse III

General Overview


This role encompasses advanced nursing responsibilities, focusing on the comprehensive assessment, care, and treatment of patients. The position may involve overseeing the work of junior staff and operates under general supervision, allowing for a moderate degree of initiative and independent decision-making.

Key Responsibilities

  • Administers medications, vaccinations, and treatments while monitoring patients for any unusual symptoms or reactions.
  • Conducts thorough physical, developmental, and psychosocial evaluations to identify patient health needs and implements suitable interventions.
  • Engages in hospital rounds or home visits to ensure continuity of care.
  • Implements and oversees case management service plans tailored to individual patient needs.
  • Provides specialized nursing services to individuals, families, groups, and communities.
  • Develops personalized service plans based on patient assessments.
  • Maintains accurate patient records and prepares program reports.
  • Plans and executes community outreach initiatives and disease surveillance efforts.
  • Contributes to the development of health education strategies and resources for clients and care staff.
  • Assists in the creation of in-service training and educational programs.
  • May participate in quality assurance evaluations.
  • May supervise the work of other nursing staff.
  • Performs additional related duties as assigned.

Qualifications

  • Graduation from an accredited four-year institution with a Bachelor of Science in Nursing (BSN) is preferred, or
  • Graduation from an accredited nursing program with an Associate Degree in Nursing (ADN).
  • A minimum of three years of nursing experience is required.
  • At least one year of experience with Electronic Medical Records (EMR) is necessary.
  • Fluency in both English and Spanish (speaking, reading, and writing) is essential.

Licenses and Certifications


Must hold a valid Registered Nurse license from the State of Texas or a state that acknowledges reciprocity through the Nurse Licensure Compact. Additionally, certification in Basic Life Support is required.


A valid Texas motor vehicle operator's license is mandatory, and the candidate must be insurable by the County's insurance provider.


Knowledge, Skills, and Abilities


Possesses a strong understanding of nursing techniques and procedures, healthcare regulations, medical diagnoses and treatments, and community health principles.


Ability to deliver patient care effectively, maintain accurate records, interpret public health laws, recognize patterns in medically necessary treatments, and supervise others as needed.
