Key Responsibilities- Implement the wraparound process to facilitate the development of the Care Planning Team
- Utilize multiple tools, including strength-based assessments and comprehensive assessments, to organize and guide the development of individual care plans and risk management/safety plans
- Work collaboratively with youth and their families in their home and community to provide support and guidance
- Develop and maintain strong relationships with community partners and stakeholders to ensure seamless care coordination
- Master's Degree in Social Work, Counseling, or Psychology, or related field, with a license in good standing (or license eligible)
- OR Bachelor's Degree in a human services field with one year of relevant experience working with families or youth
