About the Role

We are seeking a highly skilled and compassionate Care Coordinator Specialist to join our team at Family Services Association. As an Intensive Care Coordinator, you will play a critical role in providing care coordination to children with mental health needs and their families.

Key Responsibilities
  • Implement the wraparound process to facilitate the development of the Care Planning Team
  • Utilize multiple tools, including strength-based assessments and comprehensive assessments, to organize and guide the development of individual care plans and risk management/safety plans
  • Work collaboratively with youth and their families in their home and community to provide support and guidance
  • Develop and maintain strong relationships with community partners and stakeholders to ensure seamless care coordination
Requirements
  • Master's Degree in Social Work, Counseling, or Psychology, or related field, with a license in good standing (or license eligible)
  • OR Bachelor's Degree in a human services field with one year of relevant experience working with families or youth
