Role Summary:

The Personal Care Assistant is accountable to the Clinical Manager and is expected to operate as an integral member of the healthcare services team. This role is specifically trained to deliver personal care and associated services within the home environment. The Personal Care Assistant is designated to particular clients and operates under the guidance, instruction, and oversight of the Staff Registered Nurse/Case Manager, Supervising Therapist, and Clinical Manager. This position is classified as non-exempt.

Essential Qualifications:

  • Current certification as a Home Health Aide in Michigan.
  • Successful completion of a certified training program, including a minimum of 75 hours of classroom and supervised practical training.
  • At least 1 year of experience as a Home Health Aide in a healthcare or home care environment.
  • CPR certification is preferred but not mandatory.
  • Possession of a valid driver's license and current automobile insurance, along with reliable transportation.
  • Current TB test, Chest X-ray, or Skin Test (within the last 12 months).
  • Exceptional communication skills.
  • Adherence to accepted standards and practices.
  • Ability to meet the physical demands of patient care, including ambulation and assistance with exercises.
  • Self-motivated with the capability to work independently.
  • A strong team player.
  • Consistently maintains a positive demeanor that fosters teamwork and optimal performance.
  • Must demonstrate empathy, nonjudgmental attitudes, tact, responsibility, adaptability, organization, and commitment to the agency's mission.

Accountabilities:

By adhering to the Home Health Aide Care plan and/or assignment form, completed by a Registered Nurse or other qualified professional, the aide delivers services as prescribed by the physician, included in the Plan of Care, permitted under state law, and consistent with Home Health Aide training, which includes:

  • Maintaining a clean, healthy, and safe environment.
  • Assisting patients with ambulation, employing safe transfer techniques.
  • Supporting patients with nourishment and hydration needs, including meal preparation and feeding.
  • Monitoring and recording vital signs such as temperature, pulse, and respiration.
  • Implementing basic infection control measures.
  • Following the Home Health Aide care plan diligently.
  • Recognizing emergencies and applying appropriate emergency procedures.
  • Encouraging patient independence in accordance with the nursing care plan.
  • Providing emotional and psychological support to patients and their families.
  • Building trust and confidentiality with patients and their families.
  • Complying with privacy and security standards as mandated by HIPAA and other applicable laws.
  • Collaborating with personnel from other agencies involved in the patient's care as directed.
  • Exhibiting strong communication skills, including the ability to document and report clinical information effectively.
  • Actively participating as a member of the interdisciplinary team during care conferences.

Responsibilities:

  • Executing all functions outlined in the Home Health Aide Assignment Sheet/Plan of Care.
  • Providing or assisting patients with personal care tasks such as bathing, grooming, dressing, and oral hygiene.
  • Assisting patients with positioning, transfers, ambulation, and prescribed exercises.
  • Administering or assisting with bowel and bladder care, including the use of bedpans and perineal care as ordered.
  • Delivering care, treatment, and services in accordance with established standards and regulations.
  • Utilizing safe techniques for personal hygiene and grooming tasks.
  • Identifying and reporting changes in patient skin conditions to supervising professionals.
  • Assisting with treatment procedures, including simple dressing changes and preventative care.
  • Recording vital signs and maintaining clinical documentation as per agency policies.
  • Reporting any suspected cases of abuse, neglect, or non-compliance to the appropriate authorities.
  • Preparing for the patient's discharge from services.
  • Communicating with the Case Manager/Clinical Manager regarding patient services and any changes in care plans.
  • Submitting clinical documentation of services provided in a timely manner.
  • Reporting any changes in patient condition to the supervising nurse or appropriate professional.
  • Completing assignments as directed by nursing staff and reporting progress.
  • Engaging in orientation, in-service training, staff meetings, and case conferences.
  • Participating in the agency's emergency response and quality improvement programs.
  • Recognizing responsibilities related to patient rights and property.
  • Notifying the Clinical Manager of any changes in scheduling due to personal circumstances.
  • Accepting additional assignments during staffing shortages or emergencies.
  • Complying with all relevant laws, regulations, and agency policies.
  • Organizing daily visit schedules to optimize travel efficiency.
  • Maintaining compliance with OSHA and infection control policies.
  • Performing other duties as assigned.
