Role Summary:
The Home Health Aide works under the guidance of the Clinical Manager, functioning as an integral member of the healthcare team. This role involves delivering personal care and related services in a home setting. The Home Health Aide is assigned to specific clients and operates under the direction of the Staff Registered Nurse/Case Manager, Supervising Therapist, and Clinical Manager. This position is classified as non-exempt.
Essential Qualifications:
- Currently certified as a Home Health Aide in Michigan.
- Completion of a certified training program with at least 75 hours of classroom and supervised practical training.
- A minimum of 1 year of experience as a Home Health Aide in a healthcare or home care environment.
- CPR certification is preferred but not mandatory.
- Possession of a valid driver's license, current automobile insurance, and reliable transportation.
- Current TB test, Chest X-ray, or Skin Test (within the last 12 months).
- Strong communication skills.
- Adherence to accepted standards and practices.
- Ability to meet the physical demands of patient care, including ambulation and transferring patients.
- Self-motivated with the capability to work independently.
- A collaborative team player.
- Consistently demonstrates a positive attitude that fosters teamwork and optimal performance.
- Must exhibit empathy, non-judgment, tact, responsibility, adaptability, organization, and commitment to the agency's mission.
Accountabilities:
By adhering to the Home Health Aide Care plan and/or assignment form, which is completed by a Registered Nurse or other qualified professional, the aide delivers services as prescribed by the physician, included in the Plan of Care, permitted under state law, and consistent with Home Health Aide training. Responsibilities include:
- Maintaining a clean, safe, and healthful environment.
- Assisting patients with ambulation using safe techniques.
- Supporting nourishment and hydration needs, including meal preparation and feeding.
- Monitoring and recording vital signs such as temperature, pulse, and respiration.
- Upholding basic infection control procedures.
- Following the Home Health Aide care plan.
- Recognizing emergencies and applying appropriate emergency procedures.
- Encouraging patient independence as per the nursing care plan.
- Providing emotional and psychological support to patients and their families.
- Building trust and confidentiality with patients and their families.
- Complying with privacy and security standards as mandated by HIPAA and other relevant laws.
- Collaborating with personnel from other agencies involved in the patient's care as directed.
- Exhibiting effective communication skills, including the ability to document and report clinical information accurately.
- Actively participating as a member of the interdisciplinary team and attending care conferences.
Responsibilities:
- Executing all tasks outlined in the Home Health Aide Assignment Sheet/Plan of Care.
- Providing or assisting with personal care, including bathing, grooming, and dressing.
- Supporting patients with positioning, transfers, ambulation, and exercises.
- Assisting with bowel and bladder care as directed.
- Delivering care and services in accordance with established standards and regulations.
- Utilizing safe techniques for personal hygiene and grooming tasks.
- Monitoring and reporting changes in patient skin condition to the supervising professional.
- Assisting with treatment procedures as required.
- Documenting vital signs and maintaining clinical records in line with agency policies.
- Reporting any suspected cases of abuse, neglect, or non-compliance immediately.
- Planning for the patient's discharge from services.
- Communicating with the Case Manager/Clinical Manager regarding patient needs and changes in care plans.
- Submitting clinical documentation of services provided promptly.
- Reporting any changes in patient condition to the appropriate supervisor.
- Carrying out assignments as instructed and providing updates to the nurse/therapist.
- Participating in training, staff meetings, and case conferences.
- Engaging in the agency's emergency plans and quality improvement programs.
- Recognizing responsibilities related to patient rights and property.
- Informing the Clinical Manager of any schedule changes due to personal time off.
- Accepting additional assignments during staffing shortages as necessary.
- Complying with all applicable laws, regulations, and agency policies.
- Organizing daily schedules to optimize travel efficiency.
- Maintaining compliance with OSHA and infection control policies.
- Performing other duties as assigned.
