Role Summary:

The Lead Case Management Coordinator will report directly to the Chief Prevention and Recovery Officer and will oversee the Case Management functions across our services. This position is pivotal in guiding and developing our Case Managers, establishing best practices, and ensuring high standards of care. Responsibilities include facilitating team meetings, providing training for new hires, and participating in the recruitment of new Case Managers. Collaboration with other supervisory staff is essential to develop effective Case Management practices across the organization.

Key Responsibilities:

  • Implement and supervise targeted case management services that address social determinants of health (SDOH) for clients, in alignment with established criteria.
  • Conduct assessments of SDOH needs for clients upon admission and throughout their treatment journey to set collaborative, person-centered goals.
  • Ensure that client needs are met consistently throughout their treatment process.
  • Facilitate access to resources during treatment and upon discharge, particularly during critical transitions.
  • Act as a liaison with community services to assist clients in achieving their identified goals.
  • Engage with families and external resources to foster a supportive environment and ensure continuity of care.
  • Provide education on behavioral health and promote healthy resource utilization.
  • Collaborate with interdisciplinary team members to address daily concerns and intervene as necessary.
  • Assist clients in developing aftercare plans to ensure they have the necessary resources for continued support.
  • Maintain confidentiality in accordance with policies and legal regulations.
  • Ensure accurate case documentation and train staff on compliance with agency standards.
  • Identify and cultivate relationships with community resources for employment, housing, and healthcare.
  • Manage administrative duties related to staff supervision, including performance evaluations and recruitment.
  • Maintain reports related to grant initiatives and ensure compliance with funding requirements.

Qualifications:

  • High School diploma or GED required.
  • Preferred experience in addiction treatment counseling.
  • Case Management certification is required (e.g., ACM, CCM, or CPSW).
  • Current CPR/First Aid certification or willingness to obtain.
  • Valid driver's license.
