POSITION SUMMARY: In partnership with patients and their families, healthcare providers, and the interdisciplinary team, the Patient Care Coordinator is responsible for facilitating patient progression through the healthcare continuum in a manner that is both efficient and cost-effective. This role involves identifying, initiating, and managing optimal patient flow to enhance continuity of care, ensure smooth transitions, promote patient satisfaction, uphold patient safety, and manage length of stay. Comprehensive assessment, planning, implementation, and evaluation of individual patient needs are central to this position. Responsibilities also include care coordination and discharge planning. Additionally, the Patient Care Coordinator addresses payer-based inquiries regarding individual members when appropriate. This role adheres to departmental and organizational goals, objectives, performance standards, and policies, ensuring the delivery of quality patient care and compliance with regulatory requirements.

Requirements

EDUCATION:

Graduate of an accredited nursing program, with a preference for a BSN, OR a Master's Degree in Social Work (MSW) is required.

CERTIFICATION/LICENSES:

Must hold a valid Registered Nurse license to practice in the state of New Mexico, meeting all contact hour requirements for licensure, including state-mandated courses. OR Current licensure as an LMSW/LISW/LCSW in New Mexico is required. Case Management certification is highly desirable, such as ACM (Accredited Case Manager), CCM (Certified Case Manager), or RN-BC (Board Certified in Case Management).

EXPERIENCE:

A minimum of two years' experience in a healthcare setting is preferred, with at least one year in an acute care environment.

NATURE OF SUPERVISION:

Responsible to: Director of Case Management

ENVIRONMENT:

General office environment and hospital departments/nursing units (100% of the day).

Blood borne Pathogens: A

PHYSICAL REQUIREMENTS:

Ability to navigate throughout the hospital to various units/departments for approximately 80% of the day, with 20% spent in an office setting. Requires close work, good vision, and dexterity for writing and using a computer keyboard.
