WellPower - Adult Services is seeking a skilled Clinical Case Manager I/II to join our team. As a Clinical Case Manager, you will provide comprehensive psychological and behavioral case management to individuals in crisis, helping them stabilize their mental health and achieve recovery.
Key Responsibilities- Assess and support individuals in meeting their basic needs, including food, clothing, shelter, personal safety, and medical care.
- Identify individuals who require protective payee services and arrange for assistance.
- Ensure individuals have access to necessary services and community resources by arranging transportation or providing personal transportation.
- Provide comprehensive psychosocial services, including goal setting, daily living skills training, social skills development, and employment support.
- Collaborate with families, friends, landlords, employers, and community agencies to maximize benefits and minimize problems associated with the presence of individuals in crisis in the community.
- Maintain a trauma-informed environment of well-being and make full utilization of individuals' support systems to encourage treatment program engagement.
- Bachelor's (CM I) or Master's (CM II) degree in psychology, social work, or a related human services field.
- One year of experience in mental health or human services, with a valid Colorado driver's license and a recent clean driving record.
- Ability to transport individuals in your vehicle and obtain Business Use coverage on your auto policy (additional cost reimbursed).
